Sporties for Shorties On-Line Registration
Program is FREE, but registration is required
A parent or guardian MUST accompany their child(ren) in the gym during the program.
Please read the entire statement below. By completing the form and checking the box indicating your on-line signature signifies that you have read, understand, and accept the Statement of Understanding and Release for your child(ren) in participating in Sporties for Shorties in the Green Tree Municipal Center Gym. Only a parent or legal guardian may sign for a minor child.
Statement of Understanding and Release
I, the undersigned, as parent and/or guardian of the child or children listed below, hereby enroll my minor child/children in the Green Tree Sporties for Shorties Program.
In so enrolling our child/children, I recognize and agree that:
Participation in programs and recreation activities can result in serious injuries and disabilities to our child(ren);
As the parent/guardian of said child/children, I am responsible for any and all medical expenses and/or injuries sustained to my child/children while participating in the said Program;
Health insurance coverage for any child participating the Sporties for Shorties Program is not provided by the Borough of Green Tree. Responsibility for providing medical insurance, if any, is that of the parents and/or guardians;
In consideration of the Borough agreeing to accept my child/children in its Sporties for Shorties Program, I hereby release and hold harmless the Borough of Green Tree, its officers and employees, from any and all liability for any injury, disability or claim that might arise by reason of our child(ren)'s participation in the Program.
I will be present at all times while Sporties for Shorties is taking place.
Photos may be taken of my child(ren) and published in local publications, web media, and/or videos.
